BLUE CARD FORMS

New Regulations apply to all Blue Card Applications and Renewals. Please read the following information from the commission before filling in a form.

Forms are available from the Admin Office at the club and must be processed through this office.

Which Blue Card form must I complete?
I am;
A new member holding a volunteer blue card…
Authorisation to Confirm a valid blue card application’ form
A new member holding a paid employee blue card…
Authorisation to Confirm a valid blue card application’ form
A new member with no blue card…
a) A ‘Volunteer Blue Card Application’ form if… the member is a volunteer only and is not in paid regulated employment with the club or any other organisation
b) A ‘Paid Employee Blue Card Application’ form if… the member is a volunteer and is in paid regulated employment with the club
c) ‘Authorisation to Confirm a valid blue card application’ form if… the member is a volunteer and is in paid regulated employment with another organisation/employer. [They will also need to complete a ‘Paid Employee Blue Card Application’ form and lodge it with that organisation/employer*].
A current member renewing my blue card…
a) A ‘Volunteer Blue Card Renewal’ form if… the member is a volunteer only and is not in paid regulated employment with the club or any other organisation.
b) A ‘Paid Employee Blue Card Renewal’ form if… the member is a volunteer and is in paid regulated employment with the club or with any other organisation.
c) ‘Authorisation to Confirm a valid blue card application’ form if… the member is a volunteer and is in paid regulated with another organisation/employer. [They will also need to complete a ‘Paid Employee Blue Card Renewal’ form and lodge with that organisation/employer*]
*It is a requirement of the Commission that individuals fill out a form (Application, Renewal or Authorisation to Confirm as appropriate) for every organisation that they undertake regulated employment with regardless of whether it is paid or voluntary.
OTHER NOTES
Please note that current Blue Card holders do not have to transfer to a Paid Blue Card straight away, this takes places when their current Blue Card is due for renewal.
Regulated employment is employment that requires an individual to hold a Blue Card. If a member is unsure about whether their employment is regulated or not they can contact the Blue Card Contact Centre www.bluecard.qld.gov.au or (07) 3211 6999
